Critical Considerations regarding Laparoscopic Nephroureterectomy for the Treatment of Upper Urinary Tract Carcinoma
نویسندگان
چکیده
Objectives: To review the pro and con arguments for a laparoscopic approach for the treatment of upper urinary tract cancer. Methods: Fourteen series comparing open nephroureterectomy (NUT) with laparoscopic NUT have been studied with regards to cancer control and related morbidity. Results: Event though the open approach is still the gold standard treatment, the laparoscopic approach has shown its applicability with a favourable safety profile while providing good oncologic outcome. In comparative as well as noncomparative series, results were comparable for the development of local recurrences (4% vs. 6%) and distant metastases (15.5% vs. 15.1%), and patient’s long-term survival (2-yr survival: 75% vs. 76%) following laparoscopic and open NUT, respectively. However, certain types of laparoscopic bladder cuff control led to insufficient prevention of bladder recurrences. Conclusions: Laparoscopic NUT provides an oncologic outcome comparable to open surgery when performed in patients with early-stage diseases. # 2007 Published by Elsevier B.V. on behalf of European Association of Urology.

Urothelial carcinoma of the upper urinary tract: surgical approach and prognostic factors.
OBJECTIVES Open radical nephroureterectomy (O-RNU) has been the gold standard for the treatment of upper urinary tract urothelial carcinoma (UUT-UC) for decades. With the advances in laparoscopic techniques and endourologic procedures, this concept has been increasingly challenged. Oncologic outcome prediction is mainly based on stage and grade. With progress in medical treatment, adjuvant ther...
متن کاملLaparoscopic nephroureterectomy for upper tract urothelial carcinoma – Update
Upper urinary tract urothelial carcinomas (UTUCs) are uncommon and account for only 5%-10% of urothelial carcinomas. Pyelocaliceal tumors are about twice as common as ureteral tumors. Sixty percent of UTUCs are invasive at diagnosis. Radical nephroureterectomy, including the excision of the distal ureter and bladder cuff is standard of care for treatment of localized UTUCs, because of the high ...

BACKGROUND AND OBJECTIVES The aim of this study was to compare oncologic outcomes after laparoscopic nephroureterectomy, hand-assisted laparoscopic nephroureterectomy, and open nephroureterectomy for upper urinary tract urothelial cancer. METHODS Between April 1995 and August 2010, 189 patients underwent laparoscopic nephroureterectomy, hand-assisted laparoscopic nephroureterectomy, or open n...
متن کاملComprehensive Management of Upper Tract Urothelial Carcinoma
Urothelial carcinoma of the upper urinary tract represents only 5% of all urothelial cancers. The 5-year cancer-specific survival in the United States is roughly 75% with grade and stage being the most powerful predictors of survival. Nephroureterectomy with excision of the ipsilateral ureteral orifice and bladder cuff en bloc remains the gold standard treatment of the upper urinary tract uroth...
متن کاملRobot-Assisted Laparoscopic Nephroureterectomy for Transitional Cell Carcinoma of a Right Pelvic Kidney
BACKGROUND Nephroureterectomy is the standard of care for transitional cell carcinoma (TCC) involving the upper urinary tract. However, few published case reports exist describing the surgical treatment of ectopic kidneys with TCC. Surgical removal of a pelvic kidney can be complicated by aberrant vasculature supply, a tortuous ureter and abutting anatomical structures. Thus, it is necessary to...
